Online Stores
Wag: One of my faves! They don't always have the best prices or deals (except for 1st time customers), but items typically arrive in one or two days and their customer service is great. You can also add things to your cart from all of their associated "shops" (soap, diapers, vine, yoyo, beautybar, casa etc.) to help get to your free shipping total.
PetFoodDirect: Usually the best prices I can find on pet food
Doctors Foster & Smith: They offer great prices on meds and supplies, as well a variety of products to keep our animals happy. I have always had good experiences with them and would consider them a trusted site.
Amazon
Chewy
Cycle Dog: Post-consumer, recycled, non-toxic products, earth friendly products, from toys to water bowls to human apparel. Made in China though.
BigDogBoutique: Dog collars & leashes made in the U.S.A
Natural K9 Supplies
Pet360: Subsidiary of PetFoodDirect. Limited variety, but the site is personalized to your pet, which is nice.
PetFlow: Crazy cheap deals daily & they also have the "Spoiled Rotten" subscription box
PetCareRx: Great prices on pet meds

Subscription Boxes
BarkBox ($29/mo): 10% to rescue groups - Discounts for 3 & 6 mo signup. I only ordered this for 3 months because I had Groupon and suspected it wasn't worth the price without. I certainly don't think it's worth the money, but the products are all good quality and it's fun to get a box of goodies for the dog every month. My main complaint is that it's all different treats + 1 toy per box. I would much rather have a variety of toys or other products that will last. It seems like all pet boxes are like this. So many dogs are obese as it is and it's hard on sensitive tummies to have all these different foods, so I just have treats to last a lifetime now.
PetBox ($39/mo): You can pick the items you want in your box or elect to be surprised - quality, healthy brands - they donate food to rescues - for cats or dogs - discounts for 3,6, or 12 mo signup
Spoiled Rotten Box by PetFlow ($29.99/mo): Discounts for 3,6, & 12 mo signup. From other reviews it looks like this box may contain more than just treats galore, so I might have to give it a shot...
Possible discount code:
Treats & Trends ($34.95): For cat, dog or both!
Possible discount codes:
Happy Dog Box ($25/mo): Discounts for 3 & 6 mo signup - All "natural" treats, toys, etc.
Pets Love Toys ($19.95/mo for sm/med, $24.95 for lg/xl PLUS $4.99 shipping): They put together 12 gift box options for you to choose from each month. I have not tried it, but it doesn't look like a good deal from the boxes I looked at and there's no surprise either, so it's really no different than buying a bunch of stuff from a cheaper place. They do have "toy boxes" though, so it's not just all about treats!

Doggyloot: supposed to be good deals on doggy goods, kind of like Groupon. I haven't found any good deals so far...
